The case

The situation

Three teeth gone from the lower left — a first molar lost to a cracked cusp around 2013, then the second molar, then the premolar in front of them. A chrome partial denture had been made in 2018 and worn for perhaps a fortnight.

She was eating entirely on the right side and had been for years. The upper left teeth, with nothing to bite against, had drifted down into the space, which is what teeth do when they are left unopposed.

What we suggested, and why

Three teeth in a row do not need three implants. Two well-positioned implants will carry a three-unit bridge comfortably, and fewer implants means less surgery, less cost and fewer places for plaque to gather.

The scan showed enough bone height above the nerve at the front of the gap but not at the back, where the ridge had shrunk. That set the implant positions: one at the premolar site, one in the middle of the space, with the rearmost tooth of the bridge cantilevered a short distance behind the back implant.

The overgrown upper teeth were the other problem. They had come down far enough to interfere with where the new lower teeth needed to sit, so those were adjusted and two of them crowned before the bridge was designed.

What we did: implant bridge, lower left

Both implants were placed at one appointment under local anaesthetic, taking about seventy minutes. She took two days off work and described the discomfort afterwards as less than she had expected — the phrase she used was “sore, not painful”.

Four months of healing, then a digital scan and the bridge made upstairs in zirconia. Fitted at a single appointment, with a review two weeks later and again at six months.

What it cost

£7,200: £6,400 for the two implants and the three-unit bridge, plus £800 towards the work on the upper teeth. Spread over 36 months at 0% APR.
