Privacy policy
This policy explains what Larchfield Dental Clinic does with information about you. It is written to the UK GDPR and the Data Protection Act 2018.
Who is responsible
Larchfield Dental Clinic, 27 Loy Street, Cookstown, Co. Tyrone, BT80 8PZ, is the data controller. Questions about this policy go to reception@larchfielddental.co.uk or 028 9649 6320.
The practice is registered with the Information Commissioner's Office as a data controller.
What we hold
- Your name, address, date of birth and contact details.
- Your clinical records: what we found, what we did, x-rays and photographs.
- Your medical history, including medicines and allergies.
- Notes of appointments, including ones you did not attend.
- Payment records, and NHS exemption details where they apply.
- Correspondence with you and with other clinicians about your care.
Why we hold it
Clinical records are held because we cannot treat you safely without them, and because we are required to keep them. In data protection terms the lawful basis for health data is the provision of health care under Article 9(2)(h), not consent — which means you cannot simply ask us to delete your clinical notes while you remain a patient.
Contact details are held to arrange your appointments. If you have asked for reminders by text or email, that is on the basis of your consent and you can withdraw it at any time without it affecting your care.
Who sees it
The clinicians and reception staff involved in your care. Beyond the practice, information may be shared with:
- A specialist we refer you to, with your knowledge.
- The Business Services Organisation, for NHS treatment claims.
- Our dental laboratory, where work is being made for you.
- Our IT and records suppliers, under contract, where they need access to keep the system working.
We do not sell information about you, and we do not pass it to anyone for marketing.
How long we keep it
Adult dental records are normally kept for at least ten years after your last appointment. Records for children are kept until their twenty-fifth birthday, or ten years after the last appointment, whichever is later.
Your rights
- To ask for a copy of what we hold about you, normally within one month and free of charge.
- To have factual errors corrected.
- To object to processing, or ask for it to be restricted, in some circumstances.
- To withdraw consent to reminders and other optional contact.
- To complain to the Information Commissioner's Office.
The right to erasure is limited where records are held for the provision of health care. We will explain in writing if that applies to a request you make.
